VSR Forms

There are three ways to access the forms:

  1. To view and print now, click on the left PDF Open icon
  2. To save to your computer to print later, click on the right PDF Download icon
  3. If number 2 above does not work, then your browser does not support HTML 5. You will instead have to right-click on either of the PDF icons (it does not matter which) and choose SAVE LINK AS and save to the place of choice on your computer.

for activity sign-up sheets

If you find it helpful to use these pre-formatted sign-up sheet templates for activities you want to have residents sign-up for, click on the Word icon of the template of choice.

There is room left at the top of each for pictures and text. Most are formatted for legal-sized paper since our sign-ups usually exceed one letter-sized page. There are templates for second sheets also.
